Preparing for Discharge

Preparing for discharge begins upon your admission to our center. First, all residents are evaluated upon admission. Second, we collaborate with you and your family to determine appropriate and attainable goals that take into account your needs and abilities. Third, as you rehab and recover, we help you prepare or life at homes after rehabilitation. As discharge approaches, we have you practice what you’ve learned during rehab to help alleviate the fear that often occurs at the time of discharge.

YOUR DISCHARGE TRANSITION PLAN

As you prepare to discharge from the center, we will work with you to formulate plan of care to assist you in adjusting to your new environment. Your EmpRes Healthcare team will complete a Discharge Transition Plan for you. This plan is a summary of your stay and follow-up Plan of Care. It will be provided to you and to those caring for you in the community.

NOTICE OF TRANSFER OR DISCHARGE

Except in emergency situations, you will be given at least thirty days advance written notice prior to your transfer or discharge.

RESIDENT REFUNDS

Upon discharge the center will issue any refunds to the resident or, in the event of the resident’s death, the resident’s estate within thirty (30) days, or within the time required by law, whichever is earlier. No interest will accrue on any funds required to be refunded. Resident refunds less than $10.00 will not be issued.

IF DISCHARGE IS NOT AN OPTION

Your EmpRes Healthcare team will monitor your progress throughout your stay.

There is a possibility that changes in your overall health may inhibit your ability to be discharged home immediately. We will work with you and your family to anticipate these changes, and your EmpRes Healthcare team will assist you in developing an acceptable alternative care plan. One of these options may be to remain at our center. If you decide to continue your stay with us, we will be pleased to make our home your home.
